"A simple modification to allow you to use either a dollar amount or a percentage in the table rate shipping module.
This allows you to have either a set dollar amount or a percentage for any value in the table.
For example:
25:4.0,100:15%,200:9%,1000:5%,1000000:10
An order $25 or under will be charged $4.
An order for $75 would be charged 15% ($11.25).
An order over 1000 would be charged $10.
If the weight method is used, the percentage will not be used.
