"This contribution adds a CVV field and Start Date field. Also Issue number is going to be included soon.
As CVV numbers are becoming ever more required as well as start date I decided to make this contribution. It is not perfect but I have not had any problems so far and I have got some additions to add over the next few days.
Installation is included in the file as well as all necessary files and mySQL information.
